Understanding the process
If you are a victim of a work accident, you must declare the accident to your Chief Officer within the 48 hours which follow the occurrence of the accident.
To get these forms, you can either:
- Download them here
- Collect them from the State Medical Benefits Office
Your Chief Officer must complete a work accident report. You must keep the original version of this document and make as many photocopies as necessary to give to doctors, hospitals, chemists and suppliers in order to obtain the reimbursement for the services provided or products sold by the latter.
The doctors who examine you must complete two copies of a work accident and occupational disease medical certificate. One of these copies is to be handed in to your Chief Officer.
You must deliver the “Déclaration d'accident du travail” form to your Office within the 48 hours which follow the occurrence of the accident.
