Understanding the process
Any change in your personal, family or professional circumstances must be notified to the State Medical Benefits Office (SPME) as soon as possible.
All notifications should be sent to the State Medical Benefits Office using the appropriate form, duly completed and signed.
The form may be:
- Downloaded online
- Collected from the State Medical Benefits Office
Depending on the nature of the notification, it must be accompanied by the following documents:
- A copy of both sides of a valid ID document (ID card or residence permit) which details the new address
Marriage
- A marriage certificate (long form) or a copy of a family record booklet
- A copy of both sides of the spouse's national identity card and, if he or she is a citizen of a country outside the European Union, a copy of his or her residence permit
Cohabitation agreement or civil solidarity pact
- A copy of the cohabitation agreement or a PACS (civil partnership) receipt
Divorce or separation from bed and board
- A copy of the divorce decree or the order relating to separation from bed and board
Judicial separation
- A copy of the decision of the family court or any competent judicial authority
De facto separation
- For households without dependent children: Send only the form for the notification of change of family circumstances
- For households with dependent children: Attach the completed form for the declaration of de facto separation or the cessation of marital status, which can be downloaded below
Termination of a cohabitation agreement (CVC) or dissolution of a PACS (civil partnership)
- Notarial deed for the CVC or receipt for the PACS
Death
- A death certificate and the contact details of the notary's office that is dealing with the estate, if applicable
- A certificate of nationality or a photocopy of both sides of the national identity card or residence permit for the person(s) concerned
- Original copy of full bank account details (RIB) or postal banking details showing the IBAN and BIC, signed by the insured person
- Copy of both sides of the account holder's national identity card
- Death certificate
- Medical certificate of stillbirth
You can either:
- Hand in your duly completed and signed form, together with the required documents, to the State Medical Benefits Office (SPME) between 9.30 am and 5 pm from Monday to Friday, or in it post box. No appointment is needed
- Or send your duly completed and signed form, together with the required documents to the State Medical Benefits Office by post
