The State Medical Benefits Office receive now visitors in its new offices situated at 23, avenue Albert II, BP 679, 98013 MONACO CEDEX.
The office’s reception desk is located on the 2nd floor of the Fontvieille shopping centre. The lifts and escalators to reach the reception desk can be found next to Pharmacie de Fontvieille.
The red State Medical Benefits Office letterbox is set up in the planter in front of Decathlon.
Understanding the process
In the case of pregnancy, persons insured with the State Medical Benefits Office (SPME) or the beneficiaries of a person insured with the SPME must make a declaration to this Office in order to obtain a maternity record book (carnet de maternité), which will be issued subject to the entitlement to maternity benefits being established.

- Or collect it from State Medical Benefits Office
You should get the following to complete the form, before the end of the third month of pregnancy:
- The doctor who performs the medical and obstetric examination
- The doctor who performs the ultrasound examination
- The medical test laboratory
You can either :
- Hand in your duly completed and signed form, together with the required documents, to the State Medical Benefits Office (SPME) between 9.30 am and 5 pm from Monday to Friday, or in it post box. No appointment is needed
- Or send your duly completed and signed form, together with the required documents to the State Medical Benefits Office by post
