In the event of the loss or theft of a driving licence, the holder must inform the Driver and Vehicle Licensing Office immediately and may request a duplicate.

Code P4The request for a duplicate of driving licence must be sent to the Driver and Vehicle Licensing Office by submitting the following documents:
- For Monegasque nationals:
A copy of a valid Monaco national identity card
Or a copy of a national passport with proof of address (gas or electricity bill from the SMEG, rental agreement) - For residents: a copy of a valid residence permit
- The form, duly completed and signed on the original by the holder
- This form may be:

Collected from the Driver and Vehicle Licensing Office - OR, a copy of the report of the theft, made to the Police Department or Police authorities abroad
- A recent coloured ID photo, on a light background, dated within the last 6 months, frontal view, bareheaded, of size 35x45, not scanned
- The payment, to be made by cash, cheque or postal order (clearable in Monaco or France), made out to the Driver and Vehicle Licensing Office reception desk and Visa or Mastercard bank cards only, in accordance with the current fee schedule
Any administrative procedure undertaken at the Driver and Vehicle Licensing Office by a third party must be accompanied by a mandate signed by the principal and by supporting documents showing proof of these persons' identities.
The mandate allows the representative to submit and track the progress of the principal’s application, but not to sign documents instead of the principal.
To submit your application, you must first make an appointment with an agent of the Driver and Vehicle Licensing Office.
You can request an appointment in one of the following three ways:

The staff of the Driver and Vehicle Licensing Office will be waiting for you at the time of the appointment; you should bring with you the documents listed in step 1 and your preferred means of payment.

A payment receipt (invoice) and a copy of the loss declaration form, if applicable, will be provided.
The new driving licence, marked as a duplicate, will be available within 72 hours by appointment arranged by the customer manager at the end of the interview and on presentation of the payment receipt (invoice).
If a collection date is not agreed during the appointment to submit your application, you can request an appointment in one of the following three way:

If the holder of a lost or stolen driving licence is no longer a resident of the Principality, he or she may request a certificate, to be exchanged in his or her new country of residence.
